The Clinical Coordinator's primary objective is to ensure that Council:
- Provides clinical oversight to ensure the service provides high quality, person-centred aged care services that are culturally appropriate, and aligned with participants’ goals, preferences and rights.
- Remains compliant with relevant legislation, standards, and policies including aged care quality standards.
- Identify, assess and mitigate clinical risks, implementing best practice, and risk management strategies to enhance participant safety and high-quality service delivery.
- Support workforce capability and continuous improvement, providing clinical leadership, training and mentoring to all Ageing Well staff and enhance overall service delivery within the Ageing Well team.
Key responsibilities include:
- Develop, lead and review Ageing Well clinical policies and procedures
- Promote internal and external advocacy to uphold participants rights ensuring that services and systems prioritise participant choice, dignity and autonomy.
- Foster a learning culture within the team by contributing to the development and delivery of regular training opportunities, promoting skill sharing and supporting the ongoing education of Ageing Well staff
- Ensure compliance with clinical policies, procedures and conduct clinical audits.
- Oversee complex care plans, and work in collaboration with health services/clinicians as part of a multidisciplinary team (MDT), providing a holistic care approach that provides comprehensive support tailored to the participant and their families.
About You:
To support your success in this role you will have:
- Qualification in Nursing (RN Div 1) including current AHPRA registration
- Minimum 5 years’ experience in aged care or community care
- Extensive knowledge and experience in clinical assessments and developing, monitoring and reviewing care plans aligning with participant needs and clinical guidelines
- Demonstrated experience to supervise, mentor and support staff to deliver high quality care within organisational guidelines and aged care quality standards
- Knowledge of aged care legislation, aged care quality standards and aged care reforms (desired)
- Excellent time management skills and demonstrated ability to manage complex projects and competing priorities.
- Well-developed interpersonal, oral and written communication skills
- Current Victorian drivers licence, Police Check, WWCC, First Aid and CPR (or willingness to obtain)

Salary information: $96,918.12 to $107,051.36 per annum pro rata plus superannuation.
